Below is a link to a packet of forms that you will need to both submit artwork to us and to submit orders to us.  These include pricing grids.  These forms will need to be printed, filled out and mailed, faxed, or E-Mailed as pdf's. If you are submitting digital art, please (1) burn it on a disc & send along w/ these forms ...or... (2) E-Mail the art to us & then submit the paperwork via fax or E-Mailed as pdf's.*

These are pdf files (portable document format...the standard for publishing documents on the web).  They require Adobe Reader to view. If you are unable to open these pdf links, you will need to install Adobe Reader. This free download can be accessed at:   www.adobe.com

PRINTING TIP:  When in the print screen of Adobe Reader, there will be a "Page Scaling" option.  You will want to select "Fit to Printable Area" in the drop down box.  This will insure that all of the text that was formatted to print on a single page actually prints that way.

If you cannot print these forms for any reason, just call us & we can mail them to you or fax them to you.
